The Aging Resource Center (ARC) Program

The Aging Resource Center (ARC) Program offers older adults and their families the opportunity to secure community resources and services prior to discharge from the hospital and/or skilled care facilities.

What Happens When I Contact The Aging Resource Center Program?

During your stay at the hospital or short-term rehab, an Aging Resource Specialist will visit you to talk about community resources and services. Together you can consider options and make decisions about what is needed when you return home.

The Aging Resource Specialist will also work with your discharge team to help plan for your return home.

The Aging Resource Center Program offers patients and families:

Connection to Services

• On-site assessment for seamless access to in-home services
• Follow–up services at home provided by professional care managers

Community Resources

• Home delivered meals
• Bathing and housekeeping assistance
• Information and support for caregivers
• Adaptive equipment
• Transportation
• State and federal benefit programs
• Additional community resources

The Aging Resource Center Hospital Program

The Aging Resource Center Program office is located on-site at Adventist La Grange Memorial Hospital. An Aging Resource Specialist staffs the ARC office and can meet in the patient’s room, in family waiting rooms, and in the ARC office.

The Aging Resource Specialist can also provide assistance on an outpatient basis to older adults and families at the hospital,
by phone, or by e-mail.

The Aging Resource Center Short-Term Rehab Program

The Aging Resource Center’s Short-Term Rehab Program is a recent expansion of the hospital program. The same specialized services provided by the Aging Resource Specialist at the hospital are now also available at local skilled care facilities.
The Aging Resource Center Short-Term Rehab Program currently serves older adult patients and their families by appointment at:
• Lexington Health Care of La Grange
• Meadowbrook Manor Nursing and Rehabilitation of La Grange
• The British Home
