The Aging Resource Center (ARC) ProgramThe Aging Resource Center (ARC) Program offers older adults and their families the opportunity to secure community resources and services prior to discharge from the hospital and/or skilled care facilities. What Happens When I Contact The Aging Resource Center Program?During your stay at the hospital or short-term rehab, an Aging Resource Specialist will visit you to talk about community resources and services. Together you can consider options and make decisions about what is needed when you return home. The Aging Resource Specialist will also work with your discharge team to help plan for your return home. The Aging Resource Center Program offers patients and families:Connection to Services• On-site assessment for seamless access to in-home services Community Resources• Home delivered meals Health Information and Educational MaterialsThe Aging Resource Center Hospital ProgramThe Aging Resource Center Program office is located on-site at Adventist La Grange Memorial Hospital. An Aging Resource Specialist staffs the ARC office and can meet in the patient’s room, in family waiting rooms, and in the ARC office.
