Aging Well

About Us

Aging Well, a program of Aging Care Connections, is dedicated to working with communities to create an environment in which people can age well. Serving 20 communities in the Lyons, Riverside, and south Proviso Township areas, the program is composed of hundreds of older adults and more than 130 community based organizations working together to create an environment in which people can age well.

Community Action Teams

Aging Well volunteer Community Action Teams are in communities throughout Lyons, Riverside, and south Proviso Townships. The teams include older adults and individuals from community based organizations working together to address aging issues in their local communities.

Major Accomplishments

  • Community programs directed at senior issues
  • Development and distribution of File of Life packets
  • Distribution of cell phones for 911 emergency use
  • Health and Safety fairs targeted for seniors
  • Intergenerational programming with local schools

Get involved

  • Join a Community Action Team
  • Distribute information about senior programs and services to the community through the Ambassador Program
  • Coordinate a community presentation on local senior issues
  • Plan a team project to benefit seniors in your community
  • Staff an exhibit table at a local senior health fair

Workgroups

Workgroups are composed of older adults and representatives from community organizations. They help develop and plan programs in the community that address senior needs. In addition, they coordinate with the Aging Well Community Action Teams and Aging Well partners to implement projects.

Partner Organizations

The partnership currently includes hundreds of older adults and representatives from over 130 community-based organizations. Partners participate on workgroups and Community Action Teams, provide feedback, input and support the project mission for their communities.

Community Advisory Council

The Aging Well Community Advisory Council provides leadership for the initiative and is composed of community members who live and/or work in the Lyons, Riverside, or South Proviso Townships.
