Our CommunitiesArea ServedAging Care Connections covers a twenty-two community service area located within Lyons, Riverside and south Proviso Townships. The boundary lines of the agency are: 87th Street on the south, Roosevelt Road on the north, Harlem Avenue on the east and County Line Road on the west. The following communities are included within this area:
Driving DirectionsFrom the North: Take La Grange Road South past Ogden Avenue. Turn right on Harris Avenue. Aging Care Connections is on the right hand side of the street. For more detailed directions and a map of the area, you can visit mapquest here: Mapquest.com Public TransportationMetra: On the Burlington Northern line, exit the train at the La Grange Rd. stop. Cross the tracks at La Grange Rd. Walk one block South, turn right onto Harris Avenue. Walk one block west and Aging Care Connections is on the right. PACE: On the #330 line, exit the bus at the La Grange Rd. and Harris stop. Walk one block west and Aging Care Connections is on the right. Top of page Senior Bus TransportationLyons Township offers bus service to seniors who live within the Township. For information about the bus service, please visit the Lyons Township web site www.lyonstownshipil.gov and click on “OFFICES” and select “SENIOR TRANSPORTATION.” Please note that in order to use the bus service, a free pass must be secured at Aging Care Connections, Monday-Friday 8:30 a.m.-4:30 p.m. For those seniors who live in Proviso and Riverside Townships, there is a discounted cab voucher program available through Aging Care Connections in cooperation with PeopleCare, a nonprofit partner located in Riverside. To participate, seniors (60+) must:
Aging Care Connections receives Federal and State funds to help support this program. However, this funding only supports a portion of the service. Your contribution to the cost of this service is very important to maintaining it. We appreciate any support you are able to offer and encourage you to be as generous as possible with your voluntary contribution. Voucher coupon books will be distributed on a monthly basis throughout our fiscal year (October 1, 2011-September 30, 2012) based on available funding. For comprehensive information about transportation options for seniors within our service area, please call Linda Tomko, Aging Care Connections Information & Assistance Specialist, at 708-354-1323, ext. 215. Please note: To facilitate entrance to our building, there is a handicapped accessible entrance on the west side of the building. |
